La implementación del rol de la Enfermera Gestora de Caso para la profesión en Paraguay

La Licenciada Susana Díaz se desempeña como Enfermera Jefa del Departamento de Enfermería del Instituto Nacional del Cáncer (INCAN) desde el año 2014; cargo que ha obtenido por concurso de méritos y aptitudes en dos periodos (2014 y 2019).

Refiere que el INCAN es el centro de referencia de Paraguay en atención integral de personas con cáncer, según el diagnóstico situacional y los informes proporcionados por el departamento de estadísticas de la institución. Los tipos de cáncer con más demanda de atención entre el 2011 y 2020, fueron el cáncer de mama, cuello uterino, próstata y cáncer de colon.

Ante la propuesta de la APE, y realizando un análisis de las estadísticas y la gran demanda de atención en la institución, surgió la necesidad de aplicar una nueva estrategia para mejorar la atención en el proceso de la enfermedad. Es necesario mencionar que el/la Enfermero/a gestor/a de casos oncológicos (EGCO) es el punto de contacto inicial para un paciente que ingresa al sistema de salud, este profesional asegura la programación oportuna de la primera cita, la coordinación de toda la atención posterior y brinda apoyo al paciente y su familia durante todo el proceso de atención; opera como enlace entre los pacientes, las familias, los cuidadores y el equipo de atención multidisciplinario.

La capacitación se realizó gracias a un esfuerzo colaborativo de la Asociación Paraguaya de Enfermería y la Red de Enfermería de Práctica Avanzada para Latinoamericana y el Caribe, quienes convocaron a sus miembros expertos de Chile, Brasil y E.E.U.U. para realizar este entrenamiento.

Es importante destacar la aceptación y gran predisposición de la Dirección Nacional de Enfermería, y la Dirección General del INCAN, junto a su Departamento de Enfermería y el empoderamiento de los profesionales asistenciales designados.

En relación al significado del curso, la Lic. Susana refiere que le permitió conocer a profesionales expertos en práctica avanzada. Además le dio la oportunidad de crecer científicamente en la profesión, al integrar la formación de una nueva modalidad de gestión y nuevas estrategias de atención que se está implementando como plan piloto, igualmente permitió desarrollar habilidades de liderazgo, relaciones efectivas interpersonales y habilidades organizativas, afianzando el compromiso, la responsabilidad y garantizando las mejores prácticas de atención.

En lo institucional,es una gran oportunidad para el profesional de enfermería para demostrar su capacidad de trabajar en forma autónoma y en equipo. Se busca con este rol de enfermería, fortalecer la gestión clínica y la optimización de recursos en los servicios de oncología, en la búsqueda de los mejores resultados para el paciente y su familia.

Una gestión eficiente, facilita la habilidad de los equipos para enfrentar las demandas de un sistema en cambio e innovación constante, mejorando sus resultados, aportando colaborativamente desde cada disciplina, generando sinergia hacia un cuidado integral, efectivo, eficiente y centrado en las necesidades del paciente.

La gestión de casos es un nuevo rol de la enfermería, demuestra que la capacidad y el acompañamiento de enfermería pueden lograr la adherencia al tratamiento de los pacientes oncológicos, facilitando su manejo terapéutico y apoyando de manera integral al paciente y su familia.

En cuanto a proyecciones, en este momento se encuentran en la segunda etapa de la capacitación y la implementación del proyecto de Enfermera Gestora de Casos, como plan piloto hasta diciembre del año. Posteriormente se hará una evaluación y análisis de los resultados, para subsanar las brechas, dificultades, barreras encontradas, y una vez fortalecidas se considera la posibilidad de expandir en otras instituciones del país, donde se realiza asistencia a pacientes oncológicos. Así mismo, la función de EGC constituye una apertura para el desarrollo de nuevas competencias en el campo de la enfermería en el país.
