Soy David Cáceres, licenciado, egresado de la carrera de Enfermería de la Facultad de Enfermería y Obstetricia de la Universidad Nacional de Asunción (UNA) funcionario del MSPYBS, cumplo la función de agente comunitario en la USF, Policlínico Sur de Fernando de la Mora, ubicado en el barrio Cerrito de esta ciudad. Estamos instalados en el predio del dispensario municipal, Policlínico Sur que pertenece a la Municipalidad de Fernando de la Mora, que se encarga del área administrativa y limpieza.
Accedí a este puesto hace 2 años, gracias a que concursé precisamente para esta área. Con este puesto puedo realizar lo que realmente me apasiona y es, el estar en una comunidad, con la gente, y compartir, como uno más, de la familia con ellos, gracias a Dios desde el momento que gané el concurso y me presenté en la unidad fui aceptado por mis compañeras y por la comunidad en general.
En todo este tiempo he podido comprobar que mi Universidad me preparó perfectamente para esta área, para Salud Pública, un área que hace que la salud tenga otra perspectiva y otra manera de vivirla, el pasar tiempo casa por casa todos los días y que te reciban de la mejor manera, a veces con terere, a veces con alguna tortillita, pero a pesar de la humildad y precariedad en la que vive la población siempre te respetan y te dan el valor como persona y profesional. Es cierto, existen también personas que nos cierran las puertas o también, estamos expuestos al maltrato y también al peligro ya que, la zona en la que estamos desempeñando nuestras funciones, es un área vulnerable, en donde las personas tienen pocas posibilidades de trabajar o estudiar, con mucha carencia en lo material, como en lo afectivo.
Lastimosamente, es un lugar donde muchos jóvenes están involucrados en hechos delictivos y también, en el consumo de drogas específicamente el crack, una sustancia altamente dañina y destructiva, pero a pesar de eso, no nos quedamos quietos, salimos y damos lo mejor de nosotros.
He entendido que la APS aparte de ser una forma de trabajo, es un estilo de vida, todos los días aprendes algo nuevo, ya que estamos todo el tiempo en la calle, llueva, salga el sol, haya tormenta, haga frío, siempre estamos al llamado de la población.
APS es una estrategia, en donde un equipo trabaja con una población asignada con un territorio determinado, lo que hace que el relacionamiento con los usuarios sea permanente, y en el que se crean vínculos más allá de lo meramente profesional y surjan sentimientos de ambas partes. Creo que eso hace la diferencia entre trabajar en un servicio y trabajar con una comunidad, allí donde vive.
El Equipo de Salud de la Familia se distribuye entre cada uno de sus componentes, cada uno de los programas que lleva adelante el Ministerio de Salud, del cual cada miembro del equipo está encargado, y dicha distribución está encabezado por la Dra. Liz Barreto que es la jefa y encargada de las consultas pediátricas y salud familiar, también está encargada del programa PANI.
La licenciada Jazmín Vera es la encargada de las consultas ginecológicas y realización de Pap y Colposcopia y el programa de ITS, junto conmigo llevamos la responsabilidad del área de farmacia, ya sea carga y control de insumos, como la entrega a los pacientes.
La técnica en enfermería Rossana Ferreira, es la encargada del área de PAI y del programa de tuberculosis, epidemiologia, enfermería y control de residuos patológicos, somos tres los agentes comunitarios en la unidad. La Lic. Norma Silvero que es la encargada de preparar los informes mensuales, el control de censo y está encargada del club de embarazadas, y apoyo en PAI, la Lic Albina Oviedo es la encargada del programa DIT y del club de niños, y apoyo en PAI, y yo me encargo del club de HTA y diabetes, además del programa de dichas enfermedades crónicas.
Además de estas funciones, el fuerte de los agentes comunitarios está en “territorio”, ya que nosotros somos la conexión entre paciente y las USF, estamos permanentemente en contacto con la gente, en sus casas, en las escuelas, guarderías, instituciones y vemos de cerca cuáles son las necesidades de la gente, el estado de vacunación de los niños, el control de embarazadas, pacientes hipertensos y diabéticos, control constante de pacientes con tuberculosis, también damos apoyo psicológico (salud mental) y contención y mediación cuando ocurren conflictos ya sea en la familia misma, como entre vecinos..
Cabe destacar que contamos con un consultorio odontológico a cargo de dos odontólogas que cubren las guardias en la unidad, la Dra. Patricia Vega Carduz que es funcionaria del MSPYBS y la Dra Juana González que es funcionaria de la Municipalidad de Fernando de la Mora.
Lo que nos caracteriza como equipo es que más que compañeros de trabajo somos una familia, donde podemos reír cuando hay que reír y llorar cuando hay que hacerlo, si uno necesita, ahí estamos siempre para apoyarnos y salir a flote juntos como familia. Si uno está mal, todos estamos mal y si uno está feliz también, todos lo estamos.
Nuestra unidad tiene un alcance total de 8 barrios los cuales son: Cerrito, 20 de marzo, San Cayetano, San Francisco, Itakaaguy, Conavi, ko’epyahu, Concepción, aproximadamente 4500 personas que están en nuestro censo poblacional.
En tiempos de pandemia a causa del Covid-19 toda nuestra forma de trabajar cambió drásticamente, nos limitamos más a salir, en atender a los pacientes en la unidad, y varias otras cuestiones que hicieron que la población en ese tiempo esto disconforme con nuestro trabajo, ahí es donde nos empezamos a innovar en nuestra forma de dar esa atención a los pacientes, que hoy en día ya se encuentran nuevamente satisfechos en sus necesidades sanitarias.
También, hago mención de que en todo este tiempo de vacunación estuvimos al pie del cañón apoyando en los vacunatorios del Polideportivo Municipal de Fernando de la Mora y al vacunatorio del Paseo 1811, donde, hasta hoy lo seguimos haciendo.
Una de las cuestiones negativas en esta área es el abandono del que muchas veces somos víctimas los “Apesistas”, ya que en momentos de dificultad, o de necesidad siempre somos los primeros en ser llamados y los últimos en ser reconocidos, difícilmente escucharan o leerán algo importante sobre APS, pero lastimosamente siempre hay trabas y más aún, cuando de agentes comunitarios estamos hablando. Sería interesante e importante para nosotros los “Apesistas” que el Gobierno nos dé la importancia y el valor que nos merecemos.
